Empty shopper fields on add ship-to address

Hi, I'm struggling with the shopper fields on my live website (VM 3.0.4).

The fields are ok in the OPC 'bill to' form. But in the OPC 'Add shipping address' view, the fields 'title' and 'country' are empty (see screendump).
In the back-end, the fields are there and can be edited.

The settings for the shopper fields Title and Country are:
required = yes
published = yes
show in cart form = no
show in shipment form = yes
show in account maintenance = yes

Not sure if this is a OPC issue or VM3. I upgraded my development site to VM 3.0.6., there the behaviour of these fields is the same.

P.S. I noticed that it only happens with logged-in users.

It turned out to be a template thing. Element width was set to 0px.
